Referral Process and form 

Housing Stabilization Services (HSS) offers assistance to individuals aged 18 or older with a disability and who are covered by MA. Referrals can be made through their waiver case manager. If they don't have a waiver, they require a professional statement of need and a housing-focused person-centered plan. This plan can be completed by a targeted case manager, care coordinator, or housing consultant if needed.

Once referred, MCIL will register the individual with DHS. Upon approval for services, a member of the Housing Stabilization team will arrange an intake meeting with the individual.
